Required Forms
All middle school and high school student-athletes will need a current athletic physical. An athletic physical is required each year and is valid for 395 days. The physical must be on file prior to participation in tryouts, practices and/or games. Either the IESA/IHSA Pre-Participation Physical Exam Form (4 pages) OR The Illinois Child Health Form (2 pages) are acceptable. The form must be completed and signed by a physician, parent/guardian and student-athlete. Once completed and signed, the physical form must be uploaded to your daughter’s account on Willows Web in a PDF.
This Sports Medicine Form needs to be completed for each student-athlete. It can be filled out one time for the entire school year.

Middle School Transportation: Willows Academy does not provide any transportation for middle school athletics. All middle school athletes are responsible for getting to and from all athletic practices and games/meets/events on their own.
High School Transportation: Willows Academy does not provide any transportation on the weekends. All high school athletes are responsible for getting to and from meets/games on their own on the weekends. All high school athletes are responsible for getting to and from all HOME meets/games/practices on their own.
TRANSFER STUDENTS (Only Grades 10-12)
If your daughter is a transfer student in grades 10-12, please contact the Athletic Director, Rachel Meiner, at 847-824-6900 x. 247 or meiner@willowsacademy.org to schedule an appointment to review the IHSA rules and athletic eligibility guidelines. An official ruling from the IHSA must be completed prior to participation in any athletic tryouts or practices.
INTERNATIONAL STUDENTS
High School International Students are not allowed to participate in high school sports following IHSA policy. They are allowed to participate in our club sports – Golf & Bowling.
Middle School International Students are allowed to participate in middle school sports provided that they are enrolled at Willows for the entire season of the sport.
